**Picture two ways to cross the ocean.** One is a cruise ship with 3,000 passengers, a buffet that never closes, and a crew you'll never see twice. The other is a sailboat with six people aboard. On the sailboat, the captain knows how you take your coffee. She also notices when you're off your game, because she's ten feet away.

But a small boat feels every wave.

That's the trade-off behind residential assisted living, the small-home model operators are betting on hard in 2026\. Instead of a 200-unit building with a lobby and a printed activities calendar, these are licensed homes, often on ordinary residential streets, caring for anywhere from a handful of people to about 20\. The CDC counted roughly 32,200 residential care communities nationwide as of 2022.

[Senior Housing News](https://seniorhousingnews.com/2026/04/10/small-homes-big-opportunity-inside-the-growth-of-residential-assisted-living-in-2026/?ref=smartseniordaily.com) recently interviewed the people running these homes, and the pitch barely varies. **More staff per resident. More personal care. Less institution**. Examples...

- St. Louis-based **Harmony Homes** says each of its caregivers looks after four to eight residents.
- **Wellpointe**, which the National Investment Center for Seniors Housing and Care calls California's largest residential assisted living provider, keeps a floor of one staffer per six residents and goes as high as three per six when care needs climb.
- **Aroha Memory Care**, a memory care operator in Santa Rosa, California, told the outlet its residents have needed less medication and taken fewer falls since moving in. (That's the operator's own account, not a published study.)

**Wellpointe CEO George Kutnerian** put the appeal in one line: 

> "People may like to visit a resort, but they want to live in a home."

Fair enough. Here's what gave us pause.

**The business is consolidating fast.** Majestic Residences of Gilbert, Arizona, bought 17 homes from Avendelle Assisted Living in February and now runs 34.

**Brandon Schwab, CEO of Shepherd Premier Senior Living**, told SHN he expects more deals like that as staffing shortages and regulation squeeze smaller players. And **Loe Hornbuckle**, who runs Dallas-based **The Sage Oak,** said the gap between the best operators and the worst is wider now than it's ever been.

## Straight to the Point

SSD wanted to make things as simple as we could for our readers, we asked [**Dr. Carol McKinley**](https://www.smartseniordaily.com/how-do-i-know-its-time-to-start-considering-senior-living/) **what families should actually ask**. She's president and CEO of **Simpson, a Pennsylvania retirement system with three communities**, and a recipient of LeadingAge Pennsylvania's Paul P. Haas Lifetime Achievement Award. Her list is short. It's also everything the brochure leaves out.

### Dr. McKinley's Small-Home Checklist

Eight questions the brochure leaves out

1. **The real ratio**Ask for the staff-to-resident ratio on an ordinary shift, not the marketed number, and how it changes as a resident needs more help.
2. **Turnover**In a home with two caregivers, one resignation is a big disruption. Ask how long the current staff have been there.
3. **License and inspections**Get the exact license type and ask to see the state's latest inspection or survey report. Most are public record, and the home can pull them for you.
4. **The exit question**What happens if your loved one's needs outgrow what the home can handle? Will they have to move again?
5. **Nights and sick days**If the one caregiver on duty calls in sick at 2 a.m., who shows up?
6. **Itemized pricing**Small doesn't automatically mean cheaper. Ask what's bundled and what gets billed separately.
7. **Payment types**Medicaid waiver coverage for these homes varies enormously by state. Find out what they accept before you fall in love with the place.
8. **Two visits, minimum**Tour at different times of day. If memory care matters, ask point-blank how residents with dementia are handled.
